Littleton Regional Healthcare Leads Regional Community Health Needs Assessment

Littleton Regional Healthcare has partnered with other regional healthcare providers to conduct a collaborative Community Health Needs Assessment (CHNA) to evaluate the overall health needs of the region. The other organizations are Cottage Hospital, Ammonoosuc Community Health Center, and Mid-State Health Center. The CHNA offers numerous organizational and community benefits, including identifying community health needs, health disparities, and key service delivery gaps. Together, these healthcare organizations are on a mission to better understand the health needs of our greater region.

A CHNA is a comprehensive report designed to assess the overall health and wellness of a community. It helps local health systems, government agencies, non-profits, and other organizations identify the key health challenges our community faces to address the specific needs of our residents. By identifying gaps and priorities, the CHNA guides strategic planning for programs and policies that improve the health and well-being of everyone in our area.

LRH and its partners are asking community members to participate in a survey that will help them complete this assessment. The survey will run until June 30th, and your anonymous response will help these regional healthcare organizations understand diverse community perspectives, prioritize the health needs in our area, and develop action plans to address those needs. The survey is open to all community members and will take less than 10 minutes of your time

Your input will help shape the future of our community’s healthcare services and programs. By completing the CHNA survey, you ensure that your unique perspective and experience are heard. Whether you have concerns about public health or want to share ideas for improvement, your voice matters. The feedback you provide will help local healthcare organizations prioritize their efforts toward improving the health needs in our region.

The survey data will reveal the most urgent health needs and challenges in our community, such as mental health needs, access to care, and chronic diseases. These results will guide healthcare organizations in Northern New Hampshire in developing targeted strategies to address the identified health concerns. Additionally, local leaders will develop specific and measurable plans to address the issues identified by the CHNA.
